Other Services

  • (IHS) Services for people who live in their own home or family home that provide support and/or training in community living service categories. There are three types of individualized home supports:

    • Without training

    • With training

    • With family training.

  • Housing Stabilization Services is a new Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and keep housing.

  • Services provided to a person and his/her family (including extended family members) in the family’s home and/or in the community to enable the person to remain in or return to the home. This includes training of the person and family members to increase their capabilities to care for and maintain the person in the home.

  • Overnight assistance and monitoring are provided by an awake staff in the individual’s own home

  • Integra Home Health helps and prepares individuals to find the right job. Help with the job application, build a resume, and prepare for an interview. Negotiate the individual’s pay rate and benefits. Assist with arranging transportation. Provide support while employed to retain his or her position.

    Employment development services:

    Employment Services Individualized services are designed to help a person achieve competitively, integrate employment, become self-employed, or establish a microenterprise business in their community

    Employment exploration services:

    Services that help a person gain a better understanding of competitive, integrated employment opportunities in their community. Exploration activities and experiences strengthen a person’s knowledge, interests, and preferences to make informed decisions about competitive employment. Employment exploration includes: Individualized educational activities, Learning opportunities, and work experiences

    Employment support services:

    individualized services and supports that help people maintain paid employment in community businesses/settings. Employment support services occur in integrated community settings.

    Eligibility:

    To be eligible for employment services, a person must both: Receive BI, CAC, CADI or DD waiver services and have employment support service as an assessed need in their coordinated services and supports plan (CSSP).

  • We provide Personal Care Assistance. PCA is designed to help individuals with their activities of daily living. The outcome is to enable individuals to receive services that support them stay independent. Services are provided in client home at the client’s time of choosing.
